1. Scheduling

Private Consultations are by appointment only. In order to hold an appointment a credit card number is required at the time of scheduling. Appointments are available on the following days:
  • Monday 10:00 A.M., 1:00 P.M. and 3:00 P.M.
  • Tuesday 10:00 A.M., 1:00 P.M. and 3:00 P.M.
  • Wednesday 10:00 A.M., 1:00 P.M. and 3:00 P.M.
  • Friday 10:00 A.M., 1:00 P.M. and 3:00 P.M.
  • Saturday at 10:00 A.M. and 1:00 P.M. (every other week)

2. Our Program:

  1. Initial Consultation Fee: $200
  2. Follow Up Visit (30 Days) Fee: $125
  3. Lab Readings Only: $75.00
  4. Future appointments made as needed and price is determined by the length of the visit

3. Cancellation Policy:

Please note that Martin Avenue Pharmacy, Inc. must be notified by the patient at 72 hours in advance of the scheduled appointment in order for the patient to avoid a cancellation fee. If a cancellation is made with a 72 hour notice, the patient will be charged the full price of a private consultation.
