1. What is a Private Consultation?

When it comes to your health and wellness, having access to a health care professional you've come to know and trust is crucial. Even with over one million new prescriptions filled, Martin Avenue Pharmacy still offers personal attention including private consultations regarding a number of health care issues with 2003 Pharmacist of the Year, Thomas Marks.

Martin Avenue Pharmacy understands your health care concerns and is always eager to give you the attention and personal service you deserve. Private health consultations allow patients young and old to discuss their health concerns with a knowledgeable health professional, and to get help in developing a wellness plan to meet their specific needs.

In addition to discussing your health concerns in detail, pharmacist Thomas Marks, will also recommend a treatment plan. This treatment plan may include dietary recommendations, nutritional supplements, lifestyle modifications, evaluation of current drug therapy and a recommendation being sent to your current Physician or Practitioner.

Thomas Marks, R.Ph. is also trained in very specific areas of private consultation.

2. What should I bring to a consultation?

Before your appointment, please fill out and return, in its entirety, the Medical History Form. You can fill this form out online or download and print it by clicking here

In addition, if you have had or had performed any saliva tests, lab work or blood work please include a copy of the results with your Medical History Form. This information, the Medical History Form and any testing results, needs to be received by Martin Avenue Pharmacy before your scheduled appointment.

On the day or your appointment please bring with you any vitamins or nutritional you are currently taking.

4. How much time should I allow for my appointment?

One to two hours
